Incidents do happen but they are rarely advertised till some highly publicized incidents with two children of a famous celebrity were wrongly administered high doses of a drug that was many times more the total amounts they were to receive. This resulted in lawsuits and settlements but insurance carriers and Medicaid have now started to move to totally deny any coverage for expenses that arise from the mistake of hospital staff.
